CPHACC is the Capricornia Primary Health Advanced Community Care Program established to ensure best practice and improved outcomes for the local community.  It has been funded to implement world class solutions to improve the coordination of care and ensure people are not hospitalised unless other avenues have been exhausted.

The aim is to promote wellness and not simply react to chronic illness. The way we will do that is to:

• Ensure everyone knows what services are available
• Ensure the services are easy to contact
• Provide an opt-in privacy-compliant electronic record to ensure consumers have all their data where they need it in real time
• Reduce the frustration, inconvenience and cost of health care through coordinated care
